Service Contract with Carina

 

In effort to assist you in resolving your issues, and before we begin our work together, the following outlines practical material pertaining to the services I offer:

 

The first few sessions are often used for assessment of your situation and clarification of problem areas.

 

Your compliance with the treatment plan, including consistently attending sessions and actively working on your issues between sessions, will increase the effectiveness of the therapy. There will be no touching or nudity in the sessions.

 

Appointments:

Each session lasts 60 minutes unless otherwise specified. If I am late, the session will still last for the designated amount of time. If you are late, the session will end when it was supposed to end

 

Fees:

You are responsible for payment for each therapy session at the time of the session. You can also pay for several sessions in advance if you choose to do so.

 

Missed Appointments:

If you do not cancel your appointment at least 24 hours in advance, you will be charged for that session whether you are in attendance or not.

 

Confidentiality:

I will keep all information about you strictly confidential unless you give me written permission to the contray. An exception will be made where there is clear and imminent danger to an individual, to society, to you, or in the case of child abuse, which I am legally obliged to report to authorities.

 

Termination:

You have the right to terminate participation in therapy or supervision at any time, for any reason, and without any penalty. However, it is important to discuss any desire to terminate with me in advance so that you would have an opportunity to have a closure.
